E-Invoice Request Form


This form is for customers who require an official E-Invoice for purchases made with Teo Mah Poh Plastic Industry Sdn Bhd.


It applies to all purchase channels, including:


  • Direct factory purchase
  • Sales representative orders
  • Delivery orders
  • Online transactions


To comply with the Inland Revenue Board of Malaysia (LHDN / IRBM) e-Invoice implementation requirements, complete and accurate company information is required for invoice issuance.


Please ensure all details provided are correct and consistent with your company’s official registration records to avoid processing delays or rejection.


Important Information:


  • E-Invoice requests must be submitted within 30 days from the date of purchase.
  • All required fields must match your SSM and TIN registration.
  • Processing time: 3–5 working days after successful submission.
  • Once issued, any amendment may require cancellation and re-issuance in accordance with LHDN guidelines.
